Conservative, non-surgical musculoskeletal care for eligible Veterans through the VA Community Care program.

Valley Physical Medicine is proud to serve Veterans in Fayetteville and the surrounding communities. We participate in the VA Community Care program for eligible Veterans and provide conservative, non-surgical musculoskeletal care with a focus on restoring function and improving quality of life.

  • Chiropractic services are provided by Daniel J. Culliton, DC.

  • Medical services are provided by Dawn Ruminski, DO.

  • Rehabilitation and exercise services are supported by Connie Leandro, DHSc, ACSM-EP.

We accept VA Community Care referrals. Assistance with the referral process is available.


Who We Help

Veterans commonly seek care for musculoskeletal conditions such as:

  • Low back pain
  • Neck pain
  • Sciatica or radiating pain
  • Shoulder conditions
  • Knee pain
  • Hip and joint pain
  • Headaches related to neck tension
  • Activity-related strains and overuse injuries

What to Expect
Your visit typically includes a focused history, evaluation, and conservative care planning based on your clinical presentation and functional limitations. Recommendations are individualized, and care is provided within the scope of each licensed provider involved.

When appropriate, documentation can be provided to support continued conservative care based on objective findings and functional progress.

Additional Cash-Based Options

For select musculoskeletal conditions, Valley Physical Medicine also offers StemWave therapy, a non-invasive acoustic wave technology that may be used to support healing and pain reduction. StemWave is a cash-based service and is not part of VA Community Care chiropractic authorization.

If you are interested in StemWave therapy for conditions such as shoulder pain, knee pain, or plantar fasciitis, our team can discuss whether it may be appropriate as an adjunct to conservative care.

How VA Community Care Works
If you are eligible for VA Community Care, your VA provider may issue an authorization for chiropractic care. Once authorized, our office can help schedule your visits and coordinate documentation requirements.

If you are unsure whether you qualify, we recommend contacting your VA provider or VA Community Care representative for eligibility details.

Requesting a Referral
Veterans who would like to receive care at Valley Physical Medicine can request a referral from their VA primary care provider.

Helpful referral language may include:
“Please evaluate and treat for chiropractic care due to musculoskeletal pain and functional limitation.”

Continuation of Care / Reauthorization Support
If additional chiropractic visits are needed beyond an initial authorization period, our team can provide documentation that supports medical necessity based on functional deficits, objective findings, and response to care.

Common reasons for continued conservative care include:

  • Measurable functional improvement with care
  • Persistent objective restrictions affecting daily activities
  • Risk of regression if care is stopped prematurely

If you have an active authorization, you may request an appointment online or contact our office directly. Our team will help confirm your authorization details and schedule appropriately.
